Mechanism of action
Diacerein is an anthraquinone derivative with slow-onset anti-inflammatory activity. The French reference describes laboratory inhibition of interleukin-1 production and reduced collagen-degrading activity; these findings should not be presented as proof of cartilage regeneration in patients.
Pharmacokinetics
Oral diacerein is converted to rhein before systemic circulation. After a 50 mg dose, the reference reports a plasma peak near 2.5 hours. Rhein is approximately 99% protein-bound, with an elimination half-life around 4.5 hours and urinary recovery as unchanged and conjugated material. Severe renal impairment increases exposure.
